We are looking for enthusiastic and motivated clinical pharmacist to take on an exciting new opportunity working in a joint post within the acute hospital setting and a local Primary Care Network (employed by Hull University Teaching Hospital Trusts).

This is a new role and will develop over time. Within Primary Care Network you will work autonomously as part of a multi-disciplinary team, in a patient facing role including face to face and telephone consultations, supporting patients to manage their long-term conditions through structured medication reviews. You will support patients and practices with prescription and medication queries and provide clinical leadership on medication safety, medicines optimisation and quality improvement.

Within hospital setting, you will be expected to provide a clinical pharmacy service to allocated wards and offer advice to medical, nursing, other healthcare staff and patients on all aspects of the safe & effective use of medicines. This includes facilitating reconciliation, discharge planning and patient counselling, supporting clinical trials process and the safe introduction of electronic prescribing systems.

HUTH Pharmacy Department is a supportive environment for professional development with a range of experienced and specialist senior pharmacists. The Trust is keen to support professional development of the entire pharmacy workforce and you will be expected to support, motivate and engage with others. Within PCN, you will be supported by a GP clinical mentor. You will also enrol on the CPPE Primary Care Pharmacy Education Pathway and complete an Independent Prescribing Qualification, unless qualifications already held.

Not only does the city boast two Super league rugby teams, but the Championship football team of Hull City AFC also resides here. As well as excellent shopping facilities, the city centre has exceptional transport links all over Yorkshire, including trains to the nearby cities of Leeds and York. Located less than an hours’ drive to the seaside town of Bridlington, Hull is also conveniently placed to the Humber Bridge, the gateway to Lincolnshire and beyond!

The post requires weekend and bank holiday working, and includes participation in the on call service.
