We are looking for enthusiastic and motivated clinical pharmacists to take on an exciting new opportunity working in a joint post within our Acute Hospital setting and Symphonie Primary Care Network (employed by Hull University Teaching Hospital Trusts). We are offering this post as a transitional post, the successful candidate will have the opportunity to progress  to an Advanced  Clinical Pharmacist (8a) on completion of the CPPE Pathway, Independent Prescribing Qualification and demonstration of clinical knowledge and leadership . If an applicant has all the necessary skills and knowledge to meet these criteria from the outset we would consider appointing them as an advanced clinical (8a) pharmacist initially. 

This is a new role and will develop over time. Within the PCN you will work autonomously as part of a multi-disciplinary team, in a patient facing role including face to face and telephone consultations, supporting patients to manage their long-term conditions through structured medication reviews. You will support patients and practices with prescription and medication queries and provide clinical leadership on medication safety, medicines optimisation and quality improvement.

Within the  hospital setting, you will be expected to provide a clinical pharmacy service to allocated wards and offer advice to medical, nursing, other healthcare staff and patients on all aspects of the safe & effective use of medicines. This includes facilitating reconciliation, discharge planning and patient counselling, supporting clinical trials process and the safe introduction of electronic prescribing systems.

The Trust is keen to support professional development of the entire pharmacy workforce and you will be expected to support, motivate and engage with others. Within PCN, you will be supported by a GP clinical mentor. You will also enrol on the CPPE Primary Care Pharmacy Education Pathway and complete an Independent Prescribing Qualification, unless qualifications already held.

Symphonie is a Primary Care Network base in central and west Hull, serving a diverse population but with a more deprived majority. Symphonie serves a number of residential and nursing homes where the move is to have all patients in each home exclusively registered with member practices, the care of these elderly patients will be of a primary importance particularly during the on going covid19 crisis. Symphonie are a forward looking PCN and hope to develop an integrated system of care with closer clinical links to the hospital trust

The post requires weekend and bank holiday working, and includes participation in the on call service within hospital
